Bend Health Informed Consent for Clinical Intake and Neuropsychological Assessment
Evaluation Nature and Purpose of Assessment:
A Clinical Intake is a comprehensive history of an individual’s developmental and overall brain functioning. The purpose of the evaluation is to gather information on the patient's history. All the information gathered will be used to determine your appropriateness for the Neuropsychological Testing Services through Bend Health.
A neuropsychological evaluation is a comprehensive assessment of an individual’s overall brain functioning by examining cognitive domains, including intellectual functioning, language, memory, non-verbal skills, and executive skills (attention, inhibition, working memory, processing speed, planning and reasoning, and organization). In addition to the 1:1 work investigating the individual’s neurocognitive abilities, neuropsychological evaluations also include completing rating forms to understand better the individual’s day-to-day functioning and interviews that may cover topics such as medical history, general background, academics, and social functioning. The result of the evaluation is to provide insight into the functioning of the patient, make a diagnosis when appropriate, and provide recommendations. All the information gathered will be included in a report distributed to the identified patient’s parent / legal guardian. The legal guardian may decide to release the report to other parties.
Procedure:
The patient’s Clinical Intake will occur in one video session. Once the evaluation has been completed, the team will meet to discuss the appropriateness of services.
The patient’s evaluation may occur in one or more video sessions. Once all the testing and interviews have been completed (and if your evaluation includes outside rating forms, once these are also received), a report will be completed that will discuss the findings, provide a diagnosis if appropriate, and provide recommendations (unless otherwise stated such as in the case of school admissions testing). A feedback session will be offered to the family to review the results and recommendations and allow you to ask any questions that you may have.
Permission to Record:
By paying for this service, I am giving my full permission for Bend Health to video record the evaluation and testing session(s). The recording's intent is for the Neuropsychologist, MD, or other members of Bend Health’s treatment team to observe the patient during the testing environment, to gain diagnostic clarity, and to ensure that the testing environment has not compromised the validity of the measures. No recording will be done without your prior knowledge and consent.
Limits of Confidentiality:
Information obtained during the evaluation is confidential and can ordinarily be released only with your written permission. Some special circumstances can limit confidentiality, including: (1) a statement indicating harm or abuse of children or vulnerable adults, (2) a statement of intent to harm oneself or others, (3) issuance of a subpoena from a court of law, (4) in natural disasters whereby protected records may become exposed, or (5) when otherwise required by law. Clinicians occasionally consult with other health and mental health professionals on cases; if so, they make every effort to avoid revealing the patient’s identity, and the other professionals are legally bound to keep the information confidential.
Who can provide consent:
Most state law authorizes the parent(s) or legal guardian to give informed consent for most medical decisions, including mental health assessments and treatment on behalf of the minor. If the parents are married and not separated, either parent acting alone may consent to mental health treatment for the child. However, for divorced or separated parents, we need to receive consent from the legal custodian or all legal custodians.
Court Ordered Child Protective Cases or other Legal Matters:
Bend Health’s work is focused on the best interest of children, and involvement in legal matters runs counter to this focus in many ways. I understand that Bend Health will not testify for either parental party as part of child custody or other court cases. Those with an open child abuse investigation case and/or current probation are excluded from services.
